Waiver of Liability
By enrolling in any class or program offered by Attentive Safety CPR and Safety Training, you agree to the following terms and conditions:
Acknowledgment of Risks: You acknowledge that you are aware of the risks and hazards associated with CPR, First Aid, BLS, PALS, ACLS, and any other training program offered by Attentive Safety CPR and Safety Training. These risks may include, but are not limited to, personal injury, physical illness or disability, and property damage.
Release of Liability: You agree to release and discharge Attentive Safety CPR and Safety Training, its owners, officers, agents, and instructors from any and all claims, damages, or injuries arising out of your participation in any of our classes or programs, including but not limited to any claims arising out of the negligence of Attentive Safety CPR and Safety Training, its owners, officers, agents, or instructors.
Assumption of Risks: You assume all risks associated with your participation in any class or program offered by Attentive Safety CPR and Safety Training, including but not limited to the risks of personal injury, physical illness or disability, and property damage.
Indemnification: You agree to indemnify and hold harmless Attentive Safety CPR and Safety Training, its owners, officers, agents, and instructors from any and all claims, damages, or injuries arising out of your participation in any of our classes or programs.
Medical Condition Disclosure: You certify that you do not have any physical or mental condition that would hinder your ability to participate in any class or program offered by Attentive Safety CPR and Safety Training. You agree to inform Attentive Safety CPR and Safety Training of any medical condition that may affect your ability to participate in any class or program offered by Attentive Safety CPR and Safety Training.
Photography Release: You give Attentive Safety CPR and Safety Training permission to use any photographs or video footage taken of you during any class or program offered by Attentive Safety CPR and Safety Training for promotional purposes.
By enrolling in any class or program offered by Attentive Safety CPR and Safety Training, you acknowledge and agree that you have read and understand this Waiver of Liability, and that you voluntarily and knowingly waive any and all claims arising out of your participation in any class or program offered by Attentive Safety CPR and Safety Training.
