Common Court-Ordered Testing Types
Select your test and then complete the Donor Information Section. The zip code you enter will be used to determine the closest drug testing center where you will go to take your test. A donor pass/registration form with the local testing center address, hours of operation and instructions will be sent to the e-mail address you provided. Take this form with you or have available on your smart phone to provide to the testing center. No appointment is necessary in most cases. (Payment must be received at time of registration.)
